Champlain Housing Trust is the country’s largest land trust and provides permanently affordable housing to communities in Northwest Vermont. The Resident Services Department provides eviction prevention services to all CHT residents. Resident Services began as an individual staff person at CHT connecting residents with local resources, and since 2018, has expanded into a department with over 20 individuals providing eviction prevention services and essential services. During this rapid growth, the department quickly outgrew its traditional data tracking efforts, and needed a confidential and secure system that was affordable, easy to maintain, and able to hold the increased volume of data held by staff.
Shortly after submitting an inquiry on the Outcome Tracker website, we met with representatives and were so pleased to find that they took the time to learn about our agency and program to confirm the system would be a good fit before even discussing contracts. They took time to customize the system to our departments’ needs and performance metrics, and train staff on the functions of the system. Outcome Tracker was easy to learn with their help, and for almost 5 years we’ve been consistently recording data that’s helped us quantify the work we do for our Board, grant requirements, educational presentations, and more.
Without Outcome Tracker, we wouldn’t have a secure, customizable, centralized system and wouldn’t be able to share as much detail about the work we do with residents every day. It’s been truly amazing to see the volume of in person interactions quantified through Outcome Tracker. Moving forward, we’re excited to explore different functions of the system, including a public facing referral form to allow us to reach more residents who may need supports.
