“I have served as a Family Advocate at the La Habra Family Resource Center in Orange County, California for 24 years. In my over two decades using Outcome Tracker, I have consistently found it to be a seamless tool without encountering any challenges. I have had an exceptionally positive experience using Outcome Tracker. I appreciate how user-friendly it is, which makes it easy to generate reports and addi client profiles. Most notably, the program's robust data storage capabilities grant me access information on clients from previous years, effectively preventing duplication of family or client profiles when they seek assistance at the center again. This historical data allows for the continuation of seamless case management, even during transitions to new advocates in our center.

Our resource center operates under the funding of Families and Communities Together (FaCT), a network of 16 family resource centers throughout the county that work together to strengthen and support the lives of children and families. We are dedicated to service the diverse needs of Orange County residents by offering mental health support, domestic violence intervention, parenting education, external resource referrals and case management. Our mission is to “empower people to better health.” We do this by providing a complete, family-centric framework that collaborates with community stakeholders to address health-related, emotional, social, and/or academic needs. And we have received many awards for our work from different agencies such as the Orange County Supervisors Office, the city of La Habra, FaCT, and Orange County Child Protective Services. These awards reflect our unwavering commitment and exemplary service in supporting the well-being and empowerment of our community.”
